If you’ve experienced or observed a hate crime or bias incident — such as harassment, intimidation or bullying — tell us your story in the Documenting Hate questionnaire below.
Reports submitted through the questionnaire will be collected and verified by ProPublica, which is a nonprofit news organization known for its investigative, public interest-focused journalism.
They’ll then be filtered into a database aimed at helping journalists, civil rights groups and researchers better understand the prevalence of hate crimes in the United States.
